JM Supportive Services, the community support identity of JM Transitional Housing, helps individuals and families navigate California’s Self-Determination Program. Myesha Johnson has completed Independent Facilitation training and provides person-centered planning, SDP transition support, ongoing facilitation, help preparing for meetings and exploring service options, and family education. We center each participant’s goals and choices and turn complex steps into practical next actions. Based in Moreno Valley, we serve Riverside and San Bernardino counties through Inland Regional Center and offer virtual support when appropriate. Currently accepting new clients.
