Independent Facilitators

Rate: Up to $2500 for SDP Transition services to be billed under 024 Service Code, $400 a month or $100 per hour for ongoing IF support depending on case

Rate: PCP and all necessary services to transition to SDP are charged to the Regional Center, $125 per hour for ongoing IF services after transition, $100 per hour for private pay services, travel fees may apply for in-person services outside LA County, separate charge for Fair Hearing representation when applicable

Rate: $75 per hour for Independent Facilitator services, $2500 for developing the Person-Centered Plan and transitioning the client to SDP
