Independent Facilitator | Self-Determination Program (SDP)
Navigating the Self-Determination Program doesn’t have to be overwhelming. At Guided Path Consulting, I support families every step of the way, from the very beginning through ongoing SDP support.
I provide Person-Centered Plans (PCPs) and pre-transition support, including:
Budget development and spending plans
Advocacy and coordination with Regional Centers
Onboarding with Financial Management Services (FMS)
After transition to SDP, I continue to offer ongoing support, such as:
Spending plan revisions
Annual SDP renewals
Locating and allocating new vendors and services
Continued guidance and advocacy as needs change
My PCP and pre-transition services are paid for by the Regional Center. Post-transition SDP support is available at $100 per hour.
I am a courtesy vendor with six Regional Centers throughout Southern California and am fully bilingual in English and Spanish. I work closely with families from start to finish, ensuring you feel supported, informed, and confident throughout the process.
